Application Procedures

Customers can apply for e-speed services in 2 ways, they are :-

1. Fill the online application form
The procedures are as follows :-

b) Submit the completed online application form

(i) to sign the e-speed formal subscription form
(ii) to pay one time charges and subscription fee for one month in advance
(iii) to finalize other details (if any)

Note: Please bring the following when you proceed to the counter for payment
A. Government : An Original letter from Head of Department & Letter of Authorisation
B. Corporate : A copy of Business Registration or Certificate of Incorporation & Letter of Authorisation & Copies IC
C. Residential (18 years old and above) : A copy of the Customer's IC or Passport
D. Residential (under 18 years old) :
(i) A copy of the Customer's IC or Passport and
(ii) A copy of the Guarantor's IC or Passport

d) JTB or appointed installer or contractor will arrange with successful applicants to fix installation appointment

e) Billing date shall commence upon successful installation of e-speed modem at customer premises.

2. Fill the application form (hardcopy) manually (or these form can be collected from DataCom Sales Unit, JTB, Old Airport).

b) Submit the completed application form to the nearest JTB offices or fax to 02-380049.

c) Successful applicants shall be notified to come to the respective JTB offices:

(i) to pay one time charges and subscription fee for one month in advance
(ii) to finalize other details (if any)

Note: Please bring the following when you proceed to the counter for payment
